MULTIPLE GESTATION
Multiple gestations is conccurent development of two or more embryos in utero. The fertilization of two separate ova results in dyzigotic or fraternal, twins. The fertilization of a single ovum that splits and gives rise two embryos result in monozygotic or identical twins. Concurrent development of more than two fetus involves either or both the fertilization processes. Zygosity is usually established by postpartrum examination of placenta and membrances.
Management of multiple gestations: primarily bed rest aims to delay the onset of labor and the delivery of preterm infants, at least until the fetal lungs have sufficiently matured. Non traumatic delivery of viable infants are of prime concern. Maternal complications include exagerrated discomforts of pregnancy, anemia, hydramnioms, pregnancy induced or pregnancy aggravated hypertension, placenta, previa, abruptio placentae, umbilical cord accidents, preterm delivery with a complicated labor, postpartum hemorrhage, possible coagulopathy and spontaneous abortion and death of the fetus or the mother.
Implications for the fetus include congenital malformation (more common in monozygotic twins), death in utero of one fetus, shunting of blood between palcentas, resctricted intrauterine growth or discordant growth, premature birth and subsequent low birth weight and respiratory impairment.
